No, we do not bill insurance. We can only do institutional billing or patient billing. You can reach out to your insurance company to about filing for reimbursement. We cannot guarantee that your insurance plan will reimburse you.

All insurance carriers are different. You will pay for the test out of pocket at first. After the test is completed, we will send you the forms needed for you to submit a reimbursement claim to your insurance company. If you are having symptoms or have been exposed to someone that has tested positive for COVID-19 then that gives you “Medical Need” and is generally a reimbursable expense. However, refer to the FFCRA & CARES Acts for more information.

After you receive your email saying your results are ready print off the PDF, along with your receipt and registration confirmation page. Those are the forms you will need to submit to your insurance carrier. They may also have their own form you need to fill out, but you will need to contact them to ask that. Any codes you should need will be located on the forms you receive from us.

LumiraDx SARS-CoV-2 Ag Test has not been cleared or approved by FDA. The LumiraDx SARS-CoV-2 Ag test has been authorized by FDA under an EUA only for the detection of SARS-CoV-2 nucleocapsid protein. The test has not been authorized for use to detect any other viruses or pathogens. The test is authorized in the United States for the duration of the declaration that circumstances exist justifying the authorization of emergency use of in vitro diagnostic tests for detection and/or diagnosis of COVID-19 under Section 564(b)(1) of the Act, 21 U.S.C. § 360bbb-3(b)(1), unless the authorization is terminated or revoked sooner.
